Affordable home-based primary care for adults who have no health insurance and no way to get covered. Clear, low prices. No documents you don't have. No judgment.
Home visits in Worcester and Hampden Counties.
This program is for adults who have no health insurance and no realistic path to coverage — people who don't qualify for MassHealth, Medicare, or marketplace plans, for any reason — and who face real barriers getting to a traditional doctor's office. If that's you or someone you love, and no one else will see you, reach out. We'll talk it through together, in good faith.
Bed-bound or homebound adults, wheelchair or walker users, and anyone recovering after a hospital stay who is too weak to travel to appointments.
Severe anxiety, agoraphobia, depression, PTSD, or other conditions that make waiting rooms and clinic visits feel impossible.
Dementia, Alzheimer's, and intellectual or developmental disabilities where travel is disorienting or clinic settings are overwhelming.
Adults who don't qualify for coverage — for any reason — and can't find a provider who will see them. We ask no immigration questions. Ever.
People caring for a loved one full-time who neglect their own health because they can't leave the person they care for.
Adults with no way to get to a doctor's office and no one available to take them — we come to you instead.
One flat price per visit — everything the visit includes is covered. Payment is due at the time of your visit. We accept cash, card, and Zelle.
A full first visit in your home: complete health review, exam, medications, prescriptions, and a care plan built around you.
Ongoing primary care in your home: chronic condition management, medication refills, new concerns, and check-ins.
A video or phone visit for follow-ups, medication questions, and concerns that don't need an in-person exam.
We write the orders and referrals. Whether a specialist, lab, or imaging center accepts you — and what they charge — is up to them, and paid separately. We'll always help you find the most affordable option.
Our visit fee does not cover any of these, and a referral from us is not a guarantee of coverage or acceptance. We never mark up or profit from these — and we'll always point you to the cheapest safe option. See our money-saving resources below.
We did the homework for you. These trusted programs can dramatically lower the cost of medications, labs, imaging, and more — no insurance needed, and most require no SSN.
Prescriptions for a fraction of the price
Blood work at flat cash prices
The same scan for far less
Massachusetts programs many people don't know exist
Often the biggest unmet need
Support and supplies at low or no cost
These are independent programs not affiliated with Esenga Health. Prices and eligibility can change — check each program directly, or ask us at your visit and we'll help you navigate them.
Tell us a little about your situation — or your loved one's. No forms required to start the conversation. We speak with family members and caregivers too.
We'll confirm you're in our service area, explain the exact price, and schedule a home visit at a time that works. You'll know your full cost before we arrive.
Your nurse practitioner comes to your home for a full, unhurried visit — addressing all of your health concerns, with time to actually listen.
No. We do not ask for a Social Security number, and we do not ask about immigration status — ever. We only need a name, date of birth, an address for the visit, and a phone number to reach you.
Yes. Your health information is protected by federal law (HIPAA). We do not share your information with anyone outside of your care.
Good news — we accept both! If you have Medicare, MassHealth, or another plan we accept, we'll see you through your insurance instead. This self-pay program is only for people who have no health insurance and no way to get it.
If you qualify for coverage, getting enrolled is a much better deal for you than any price we could offer — it covers your medications, labs, specialists, and hospital care too. We'll help you get the application done, and once you're covered, we'd be honored to be your regular primary care provider through your plan.
No. There are no memberships, no monthly fees, and no subscriptions. This is simple per-visit pricing, and it exists for one reason: so that people who are locked out of health coverage can still have a primary care provider. It's a safety net, not a luxury service.
Absolutely. Many of our patients are connected to us by a son, daughter, or caregiver. Call us and we'll walk through everything together.
Talk to us anyway. We have a good-faith sliding scale for people in financial hardship — no documents, no proof of income, no credit checks. Our goal is that cost is never the reason someone goes without primary care.
No. Your visit fee pays for your visit with us — including writing your prescriptions, orders, and referrals. But the pharmacy, the lab, the imaging center, and the specialist each charge their own fees, and those are yours to pay. A referral from us is not a guarantee that a specialist will accept you or that the cost will be covered. What we can promise: we'll always tell you the cheapest safe option, and the resources on this page can bring those costs way down.
We prescribe what is clinically appropriate and safe for you — the same standard of care any patient receives. Paying out of pocket doesn't change our clinical judgment, and we don't prescribe controlled substances on request.
Cash, debit and credit cards, and Zelle. Payment is due at the time of the visit — and that payment is the whole bill. You will never get a surprise bill from us afterward.
We work with interpreter services and welcome patients and families from all backgrounds. Let us know your preferred language when you call.
Yes — that's exactly what this is. This isn't urgent care. We become your primary care provider: managing your conditions over time, refilling your medications, and being the one who knows your whole health picture.
Call, text, or email us. Tell us your situation — or your loved one's — and we'll figure out the next step together. No judgment. No pressure.
