Esenga Health was born from a simple insight gained over more than seventeen years of patient care: adults with chronic illnesses often fall through the cracks of the healthcare system. As a nurse practitioner trained in family, acute, and psychiatric mental health care, I have seen many patients repeatedly cycle through hospitals. These include individuals with Alzheimer's, Parkinson's, diabetes, hypertension-related complications, multiple strokes, respiratory conditions such as COPD and emphysema, spinal injuries, autoimmune diseases, heart failure, brain injuries, and behavioral health issues such as depression, anxiety, and schizophrenia, among many other debilitating chronic illnesses. A recurring pattern is evident: patients without consistent primary and mental health support tend to worsen at home until a crisis necessitates emergency or hospital care.
Esenga Health was created to break the cycle of reactive care. We focus on whole-person, relationship-based care to prevent issues, manage chronic conditions, support mental wellness, and ensure smooth care transitions. Serving adults of all ages, from young adults with complex conditions to older adults with multiple illnesses, we bring primary and behavioral health services directly to their homes. Regular, proactive visits allow early problem detection, coordination with pharmacists, specialists, and caregivers, and help patients stay stable at home. Our goal is to reduce hospitalizations, ease burdens on patients and families, and provide compassionate care to those in need.
