No. You can keep your PCP, and we'll send them our visit notes. If you have Original Medicare, you can see us without changing anything. If you have a Medicare Advantage or MassHealth plan, your plan may have its own rules about referrals or which providers you can see, so call us or your healthcare plan to confirm your coverage and any referral requirements.

As your primary care provider, we come to you, whether that's your home, a group home, or another residential setting in Worcester and Hampden Counties. We manage your ongoing health conditions and medications, see you after a hospital stay, and support your mental health. We also order equipment and supplies you need at home, refer you for screenings and specialist care, and coordinate with your home health agency, caregivers, and family so everyone is working from the same plan.
We provide primary care, chronic disease management, medication management, post-hospital follow-up, behavioral health services, lab and diagnostic coordination, specialist referrals, and care coordination.
Services are based on each patient’s individual medical needs.
Your first in-home visit typically lasts 60–90 minutes. Your provider will review your medical history, current medications, and health goals; check your vital signs; perform an appropriate physical exam; and discuss your concerns. We’ll also review preventive screenings and tests that may be recommended based on your age, health history, and risk factors. Together, you’ll begin building a personalized care plan.
When appropriate, your provider may order lab tests or imaging, prescribe or adjust medications, arrange medical equipment, or refer you to a specialist. Follow-up visits focus on your ongoing needs and care plan.
No. Esenga Health provides longitudinal primary care for adults who need ongoing medical care at home. We are not an urgent care, walk-in clinic, or on-demand house-call service.
Same-day and next-day visits are usually not available. Established patients may contact us about new medical concerns, and we will help determine the appropriate next step. Depending on the situation, this may include a scheduled home visit, telephone or telehealth support, urgent care, or evaluation in an emergency department.

The frequency of visits depends on your medical conditions, care plan, and changing healthcare needs. Many patients are seen approximately three to four times per year, with more or fewer visits when medically appropriate.
Home visits are scheduled in advance and are intended to support ongoing primary care rather than provide immediate or on-demand treatment.
Yes. Established patients may call or text Esenga Health about a new concern. When appropriate, we may schedule a problem-based visit to evaluate issues such as infections, pain, or changes in your health. Same-day and next-day appointments are not always available. If your symptoms cannot wait for a scheduled visit, go to an urgent care center or emergency department. For potentially life-threatening symptoms, call 911 immediately.
Home-based primary care is often covered similarly to care provided in a medical office. Coverage, copayments, deductibles, and other costs depend on your insurance plan and benefits.

Esenga Health provides home-based services in Worcester and Hampden counties. Our service area generally includes locations within 50 driving miles of our Worcester office, with a travel time of approximately one hour or less.
Availability depends on your specific address, travel time, and scheduling. We may not cover every town within these counties, so please call us to confirm whether we serve your location before scheduling a home visit.
Some basic testing may be completed during your visit. When available and covered by your insurance, we can coordinate mobile laboratory draws, X-rays, ultrasounds, and other diagnostic services in your home.
Certain tests may still require an appointment at a laboratory, imaging center, or medical facility.
