Our home-based services are designed for adults who may have difficulty accessing traditional office-based care. This may include individuals who:
- Have mobility limitations or difficulty accessing office-based care
- Are living with physical, intellectual, or developmental disabilities
- Have multiple or complex chronic health conditions
- Are recovering after a hospitalization or rehabilitation stay
- Have difficulty attending regular medical appointments
- Would benefit from ongoing medical care provided in the comfort of their home
You do not have to be bedridden to receive home-based care. We evaluate each patient individually to determine whether our services are appropriate for their needs.
We provide full primary care and behavioral health care services—just like a traditional clinic—but we come to you. This includes physical exams, medication management, lab orders, and ongoing care, all in your home.
Esenga Health provides real, ongoing primary care—not urgent care or simply a one-time visit. We can serve as your primary care provider, see you regularly, manage chronic conditions, and help coordinate your care over time.
We'll review your medical history, take your vital signs, perform a physical exam, and answer your questions. Based on what we find, we'll determine if labs, imaging, or specialist referrals are needed.

Often, yes. Because we provide primary care services, home visits may be covered similarly to office visits, depending on your insurance plan. We can help verify your coverage.
If you're uninsured, please ask us about our sliding scale fee. We want cost to never be the reason someone goes without care.

Yes. Basic testing like vital signs and some fingerstick labs can be done during your visit. For additional labs or imaging, we coordinate mobile services that come to your home when possible.
For emergencies, call 911 or go to the nearest emergency room. For urgent concerns, seek immediate care and then follow up with us so we can continue your care at home.
